Ideas from Tim Cera:

Numbered heads: Add "." to end. Totally reworked Html.numbered_heads, eliminating the perl leftovers. Thus, converted from ":" delimited strings for saving the head stack to true seqs of numbers.

Allow tables without captions. Reworked to eliminate 2 re patterns.

Allow proper use of spaces in C<>. All I did was eliminate the extra spaces on all the translation in Html.

Break up design history by release (with big lump covering up to roughly 1.2).
